PDC Teacher Recommendation Forms

Please have ALL current teachers fill out a recommendation form online.

You can:
A: give them the paper with the QR code and URL
B: email your teachers the URL link: http://goo.gl/forms/bq1jozsiOa

C: print off the recommendation form, give it to the teachers, have teacher fill out and send to Mrs. Di Lello in envelope. (not recommended)
